A maintenance contract format is the document which lays down the detailed description of the design in which a maintenance contract should be drafted. These types of formats can be designed for various different types of maintenance contracts which can be then used as per needs and requirements. A maintenance contract format contains information of the both the parties entering into the contracts and all the terms and conditions agreed between them.

The first part of the contract mentions the official contract number.

The second part contains the date on which the contract is registered.

This paragraph should mention details like the name of the service provider and the client along with the address of both the parties.   1. It is very important to mention the maintenance fee that the service provider will be receiving and whether he would receive it on a monthly basis or per service basis.
  2. We have to mention the contract expiry date and what are the procedures under which the contract can be cancelled before the contract expiry date.
  3. We can mention the address at which the service has to be provided and how often will the service provider provide his service.
